Pharmacy Reviews

Medication utilization, safety, and necessity reviews led by board-certified medical expertise.

Overview

CMRS’s Pharmacy Review program addresses high-cost, high-risk, and clinically complex medication regimens across Workers’ Compensation, Disability, Group Health, Auto and Liability claims. Our reviews go beyond simple repricing or formulary matching – they apply clinical judgment to questions of medical necessity, polypharmacy risk, opioid stewardship, drug interactions, off-label use, and alignment with evidence-based guidelines.

Every review is performed by board-certified medical expertise familiar with both the clinical and operational realities of insurance claims. We deliver written, defensible determinations that your claims team, attorneys, and pharmacy benefit managers can act on with confidence.

Typical Review Questions We Answer

  • Is this medication regimen medically necessary given the diagnosis and clinical course?
  • Are dosages, durations, and combinations consistent with FDA labeling and evidence-based guidelines?
  • Are there safety concerns, drug interactions, or polypharmacy issues that warrant intervention?
  • Is the regimen aligned with the treatment plan and functional goals – or is it perpetuating unnecessary care?
  • Are there lower-cost, equally effective alternatives within the therapeutic class?
  • Does the opioid regimen meet CDC, ODG, and state-specific opioid stewardship standards?
  • Is compounded medication usage clinically justified – or simply margin-driven?

Our Review Methodology

1. Clinical Triage

Our pharmacy review team triages incoming files for complexity – flagging regimens involving opioids, benzodiazepines, compounded medications, polypharmacy patterns, and high-cost specialty drugs for deeper clinical review.

2. Specialty-Matched Physician Review

Complex regimens are reviewed by physicians whose clinical practice is relevant to the underlying condition – pain management, internal medicine, psychiatry, oncology, etc. – ensuring the recommendation is grounded in real clinical context.

3. Guideline Citation

Every recommendation cites the relevant evidence-based source: ODG, CA MTUS, CDC opioid prescribing guidelines, FDA labeling, AHFS, or specialty society practice parameters as applicable to the jurisdiction.

4. Quality Assurance

Every report passes through QA before delivery – checked for clinical reasoning, citation accuracy, completeness, and direct response to your referral question.

Specific Review Programs

  • Opioid stewardship reviews – appropriateness, dosing trajectory, MED equivalents, alternative therapy consideration
  • Compounded medication reviews – clinical justification vs. commercially available alternatives
  • Polypharmacy reviews – interaction risk, deprescribing opportunities, regimen consolidation
  • High-cost specialty drug reviews – necessity, prior authorization criteria, evidence base
  • Behavioral health pharmacy reviews – appropriateness of psychotropic regimens, benzodiazepine concerns
